STEP 1:  

Download the PARTICIPANT APPLICATION FORM  below and save it to your computer.  (right click and save as)  Fill it in completely, either typing it in using your computer, or writing clearly using black ink, and when finished, send it to the Participant/Mentor Coordinator for the site you are applying to.   You can either e-mail it or send it by mail.

Please include payment for the registration fee of $130.00 with the application.  If you chose to e-mail in your application form, please print a hard copy of the last page, sign the document in the appropriate place, and mail it in along with the application fee to the appropriate address.

STEP 2:

Download the other forms below and send them on to your references, requesting that they fill them out and e-mail or mail them to the Participant/Mentor coordinator of the appropriate site as soon as possible.  Please ensure that the materials are sent to the address of the ECLDN staff at the site you are interested in participating in.

STEP 3:  

After receiving all of your application materials, an ECLDN Staff member will  contact you to set up an appointment to talk through your application.   Please be aware that your final acceptance into the program will be the decision of the entire ECLDN Staff Team only upon reviewing all of your application materials.

STEP 4:

All accepted applicants are expected to attend an Orientation Session. 

ECLDN (Ontario West) at CRC offices on Mainway.

  Year One Participants:   September 25, 2010...8:30 am to 2:30 pm.  Lunch included!                               

 Year Two and Year Three Participants:  September 25, 2010...8:30 am to 12:30 pm.  Lunch included!

 ECLDN (Ontario East) 

Year One, Two and Three Participants:  October 2, 2010...8:30 to 2:30
